1.  SPORTS ENGINE ACCOUNT:  Create a SportsEngine account or log into your existing one.  You need only one account per family.

2.  USAV MEMBERSHIP:  Sign up for a Gulf Coast Region Membership through USA Volleyball.   (see the Age Definition Chart to know exactly which membership to purchase)

3.  MEDICAL RELEASE FORM:  Complete the USAV Medical Release Form.  It must be notarized.  We will have a notary at evaluations.  You must bring a photo ID.
